Keep copies in your phones, and consider keeping copies in glove boxes, first aid kits, etc.

Some say that EMTs check your fridge, so it might be worth thinking about printing out a copy and putting it there if your elderly loved ones live alone. It's free and might make a difference.

Medical personnel might go through wallets to figure out who people are, so do consider wallet cards too. I've tried to generate these (The "Card Format" button above) so that you can cut them out, but I can't seem to perfect it. If nothing else it should give you a starting point.

Most mobile devices allow you to create PDFs of webpages. I recommend you tap/click "COPY TO CLIPBOARD" and paste this data in your notes / email etc to keep a copy first, and then look up how to create a PDF on your device.

You should also have an option to "Save as PDF" when you're printing on desktop.

Why'd you make this?

I made this because I realized something:

If something happened to my parents and I had to call an ambulance, I wouldn't be able to tell the emergency responders information they needed to know.

And it's not even a matter of just knowing it; I don't know how well I'd react under that stress. Would I forget something important? Would I even be contactable?

It was about being prepared and peace of mind.
